Think what people might have thought when John Romero was fired. After Quake3, when John Carmack wanted to do Doom3, most of them were not happy. When he said either do it or i quit, everyone agreed (though they fired Paul Steed). So nobody was ready to do yet another sequel which is why they gave Quake4 to Raven i guess.
As long as John Carmack is there, we can expect the next revolutionary game technology.

I have heard that John Carmack is working on some new renderer for a game different from Quake or Doom.
I don't thint id ppl should worry too much abt sequel as their games are more about the the feel and the technology than the story line.
